eFORMS
Our office now uses the eFORMS platform to handle updates to patient information which allows us to provide patient registration in a paperless fashion which is safer for all parties. To utilize the eFORMS platform you must make sure that you have not opted out of or blocked text messages or emails from this office (such as appointment reminders sent out by our practice management system). Please correct that as soon as possible to receive your link to register!!
PRIOR TO YOUR APPOINTMENT
4-7 days prior to your appointment you will receive an email confirmation and a request to complete or update all critical patient forms. Please follow the links to do so and then finish when prompted. The completed forms will then be sent to our mailbox where they can be opened and reviewed. Some forms can be signed online while others will need to be signed in office while we process your identification. Please remember to bring all necessary forms to your appointment including Driver’s License, Insurance Card, and form of payment.
ARRIVAL
Please arrive 10 minutes prior to your appointment and call the office to notify them of your arrival from your car. We are asking that only patients with appointments come into our office. If you have a family member(s), a driver, or anyone else with you, we kindly ask that they remain in the car, unless you require assistance. You will remain in your car until called and then proceed into the office where you will have your temperature taken, provide your identification cards, and proceed to the kiosk to review and sign your forms and complete the sign-in process. At that time you may take a seat and Angela will notify the assistants that you are ready. Please wear a mask to your appointment and keep it on until in the treatment area. If the appointment is for a child then the parent can escort them to the office and complete their forms using the kiosk and stay with them in the reception room until they are called for their appointment. No other family members are allowed in the office and that includes siblings so please consider who will bring the child before scheduling the appointment.
APPOINTMENTS
All of our providers and staff are wearing masks plus isolation gowns and we have installed plastic shields at all of our check-in and check-out windows. We have increased our cleaning protocol especially for common areas and high touch objects and have taken steps to mitigate aerosols in the treatment areas. Additional steps have been taken to mitigate aerosols which might arise in the treatment areas including Medical Grade HEPA filtration, UVC Germicidal lights, and electrostatic fogging with non-toxic Hypochlorous solution. The office closes at lunch to further disinfect operatories and common areas then again at closing.
All staff have been OSHA trained and certified in the proper use of the N95 respirators and updated guidelines from the CDC and the American Dental Association have been added to our routine. There are still limitations to the services we can provide however we will do our best to provide as many scheduling options as we deem safe for everyone.
SCHEDULE
Because of the backlog of canceled appointments due to the extended office closure, we have been scheduling patients on a priority basis starting with emergencies and patients with pre-existing cases that need finished. For all other routine and cleaning appointments, we will make every effort to have you seen in a timely fashion and would appreciate your patience. We are rescheduling our patients in the order they were canceled. We are aware of the needs for some teens to be seen before school starts or college begins in August so we will be adding days to our summer schedule to accommodate those families with students. Please be aware that periodic shutdowns may occur in our area if virus activity re-appears during the fall or winter seasons
If you have a Facebook account, please make sure to like and follow Valley Brook Dental to stay up-to-date on this ever-changing situation and how it affects the dental field.
Thank you for your patience during these trying and unprecedented times. We all look forward to life returning to normal but for now but this is the new normal and, despite the restrictions, we will strive to continue providing quality care in a safe and healthy fashion for staff and patients alike.
